Payer Modifier Rules

Structured source of truth for the payer modifier rules — replaces the hand-maintained Excel sheet; the backend computes + writes these to Candid (O3 / PE-2432 / PE-2576). Backed by the worker’s /payer-modifiers API when configured, else an in-memory stub.
